What is Long-Term Care?
Having a critical illness like a Stroke, COPD, or Cancer is challenging. Repeated long hospital stays which are common in such cases, can become unaffordable with many tests, doctor visits and multiple procedures. Providing care at home may also be difficult due to distance or lack of caregivers. Is there a place where trained and experienced nurses & physiotherapists take care of patients in a homely atmosphere? Yes. Now there is. At the heart of Indore. Presenting Care Continuum’s Long Term Care Centre – The best chronic care services in a safe and homely environment.
Where in Indore, is LTCC located?
The Centre has been created by remodelling an entire wing of Mission Hospital, Chhawni, Indore. The heritage, location, primary intent and ethics of the Hospital, which today is over 130 years old, was key to being considered a suitable existing healthcare facility which could be revived to provide loving expert care to the critically ill or infirm. People suffering from serious illnesses can stay here comfortably for a few weeks to months, while they receive good nursing care, physiotherapy and tasty nutritious food.
When do doctors refer patients to LTCC?
Any person who is suffering from chronic illness or disability and requires continuing care over weeks and months (not just a few days) can be admitted to LTCC. The following are some examples of situations where LTCC benefits patients:
- Supportive Care (Neuro, Ortho, Respiratory, Cardiac Rehab)
- Long-Term Intensive Nursing (Trauma, Surgery, Frailty)
- Palliative Care (Comfort Care for Cancer, Parkinson’s, MND etc)
- Psychiatric Rehabilitation (Monitoring)
- Respite Care (To allow family caregivers temporary relief & rejuvenation for a few days)
- Elder Care (Holistic Care Model including Cognitive Stimulation)
How are patients admitted to LTCC?
The process of admission is quick and easy. Once the doctor refers the patient to our Centre to continue care and rehabilitation, a complete handover is taken from the current hospital and the patient is discharged and brought to LTCC by our senior nurse. Depending on the patient’s condition and choice of bed, the patient is admitted to the ward or private room, and made to feel at home. Being surrounded by greenery, the Centre is like an oasis of calm and completely different from a hospital environment making patients feel at peace. All therapy is as per the direction of the principal treating physician. Our doctors provide supportive care and interact with the consultant regularly to update the treatment if necessary.
What are the services available at LTCC?
A full range of services is provided by our Multidisciplinary care team & include:
- Manual & Machine Based Functional Rehabilitation & ADL Training
- Regular and Critical Care Nursing (Including Cardiac and Respiratory support).
- Pain Management through Physiotherapy, Nursing Interventions and Medications

Is LTCC like a Hospital?
For patients and their families, our LTCC is like a step-down facility from a hospital OR a step-up facility from home, where they can be assured of the best-trained care at affordable costs. We also accept seniors who need temporary supervision and care while their family caregivers are away from work or leisure (Respite Care). In such cases, we assess the seniors and again, take a detailed handover so that their care continues just as it did at home.
What happens if a patient becomes critical at LTCC?
In case of emergencies, we immediately inform the principal consultant who then decides which facility would be best for the patient’s acute care. Accordingly, the patient is shifted from the LTCC to the hospital in the shortest possible time, if needed. Being in the heart of Indore, most hospitals are just a few minutes away and once stable, the patient can be easily shifted back to LTCC to continue the recovery.
