Care Continuum was conceived & founded in 2014 as a social enterprise, to make a significant difference to the on-demand model of Home Healthcare practised by a majority of the providers till date. The Home Team
Successful Healthcare is the effort of multiple categories of staff from the specialist Physician to the housekeeper working together. The multidisciplinary team at Care Continuum is a well-knit unit, conceived and directed by our founders. A Model unique to Care Continuum, which other agencies have failed to replicate due to the deep surveillance and retraining required.
Our Team works with a Homecare Plan made in advance by assessing the patient prior to the start of care. Team members have the following roles:
Our team physician does Initial and periodic reviews to check for response to the ongoing medicines and discusses the status with the Specialist to modify the prescription and the nursing care as required.
Our Care Co-ordinator organises for all Point of Care items such as Hospital Bed, Suction Machine, Pulse Oximeter etc. on rent or sale to be delivered home, as advised.
Our Home nurses start bedside nursing – check vitals, give medicines, detect any signs of worsening and connect with the Doctor / Nursing supervisor for further action.
Our Nursing Supervisors monitor bedside care 24 x 7 on phone & weekly visits. They also put IV lines, tubes & drains if advised. Patient & family needs are put first.
Our specialist associates are called in for services such as physiotherapy, psychological counseling, speech therapy, dietary counselling, movement psychotherapy etc.
Our back Office arranges for any Investigations advised such as Blood Tests, ECGs, X Rays etc. Care coordinators are on standby round the clock, for emergency transfers.

- General Physician (Part time) – MBBS with 5+ years of experience. Training in Geriatric / Palliative / Family Medicine preferable. Should be willing to travel for home visits within the city.
- Nurse Supervisor – GNM / B. Sc. with 3+ years OR ANM with 6+ years of experience. Experience in bedside procedures is essential.
- Bedside Nursing Staff – GNM / ANM / HHA Certification. Proof of credentials & references / practical training is mandatory.
- Eldercare Co-ordinator – Graduate Females with previous experience in working for vulnerable sections of society.
- Physiotherapist – MPT / BPT from Government recognised University. Diploma holders are accepted as Trainees working under supervision.
- Occupational Therapist – BOT from Government recognised University.
- Psychologist (Part time) – Sc. Clinical Psychology / Rehabilitative Psychology
- Care Pals / Companion – Fit Male / Female candidates over 18 yrs, with work experience of 1-2 years and adequate references may apply for Skilling / OJT.
