Hospitals have long been regarded as the only place for management of patients with illnesses that require treatment urgently, or are conditions beyond a certain level of severity, for which significant levels of resources are required. That scenario is changing rapidly.
There has been a complete transformation in the urban disease pattern over the last few decades, with chronic, lifestyle diseases and cancer replacing infectious diseases and trauma as the major illnesses. This has also altered our longevity, with a substantial number of people living well into their seventies and beyond. In turn, this has forced the medical profession to focus on the long-term management of diseases & debilities of old age. Hospitalization continues to be required for acute care, interventions and emergencies but visiting a hospital for therapy, specially of the chronic variety, is no longer an ideal solution for patients who need sustained support and easier access to non acute care.
If we look around, there already exists a community model which provides this service. The GPs or General Physicians – the family doctors have been doing just this for ages. Then why is there a need to develop a different model now? Principally, continuing care outside the hospital for today’s complex medical conditions requires dedicated teamwork and managed resources similar to that usually available at hospitals but which is better attuned to the lower levels of severity, and as a consequence, is more sustainable and affordable.
This is precisely what we at Care Continuum have set out to achieve - Providing care in settings that are less formal, more relaxed and more familiar for the patient, in a variety of formats. We have created an ecosystem within the community where the strengths of the Family Doctor working alone are complemented & bolstered to provide safe and convenient care, taking into account the patients’ & their families’ preferences and inputs.
We are committed to Redefining Care And Improving Lives...
