Safer @ Home Situations
Sometimes, the intensivist / consultant in charge of your family member, who is in the Intensive Care Unit at hospital, advises a release due to the risk of cross Winfection, high costs or other factors. worry not because we provide a comprehensive critical care for your special requirements. We have a tried and tested Home Critical Care setup, with access to a multidisciplinary team to care for and monitor your loved one.
Our Critical Care Model
Our team involvement starts with the physician’s assessment in the hospital itself. The Team doctor prepares an Intensive Care plan in discussion with the treating consultant as well as the family. This plan covers all aspects of critical care e.g. the fitment of a hospital bed at home, wheelchair or bedside commode, oxygen supply, multipara monitor, ventilator etc. Once your ward is ready for discharge, our critical care equipment team prepares the home room setup while the nursing team which has already received the care plan, reaches the hospital to take a complete handover. The patient is safely transported home in the care of our nursing and Ambulance staff and the Home Care plan is commenced under the guidance of the Team Physician in consultation with the intensivist / specialist. All aspects of intensive care nursing including state-of-the-art critical care medicine and critical care technology are taken into account.
The Team of doctors closely monitor the ongoing care at home and regularly updates the treating physician and the family members of the clinical status. In case of any decline in the condition of the patient, every attempt is made to support the patient at home through the visit of the Nursing Supervisor whenever needed. In case the situation demands readmission, our 24 hour helpline co-ordinates emergency transport to the hospital advised by the primary / family physician. To sum up, our critical care team takes full responsibility of the sick and elderly.
